Have You Moved?

If you are a current policyholder with us and would like to change your name or mailing address, please download this Address/Name Change Form (PDF) and mail or fax it to the Home Office. Using this form will change all records for account and magazine mailings.

Change of Beneficiary?

If you are a current policyholder with us and would like to change your beneficiary information on your certificate, please download the Election of Change of Beneficiary Form (PDF) and mail it to the Home Office. Using this form will supersede all existing beneficiary(ies) information on the certificate.
