20 Pay Life ICC19WL
Issue ages 0-75, Minimum face amount $5,000
Great option for insureds looking for slightly less annual premium than the 10 Pay WL but still want a short pay policy
- Younger people who realize the need for permanent life insurance but don’t want to pay premium for a lifetime
- Business owners who want to fund key person or deferred compensation plans
- A good policy for parents to purchase for their children. The policy is paid up by the time the children graduate from college and are starting a family and career.
Whole Life – Lifetime Pay ICC19WL
Issue Ages 0-75, Minimum face amount $5,000
- Accumulates savings in cash value and provides a lifetime of protection
- Good product to accumulate money to supplement retirement savings
- Can be used as a final expense policy with the low face amount available

Issue ages 0-23 for annual premium and 0-19 for single premium. Face amounts $10,000-$50,000
- Great term product for children and young adults
- Convertible with a conversion credit at age 30 to any permanent product up to face value

20 Year Level Term ICC19TREM
Issue ages 18-75, Face amounts $25,000 to $100,000
- Very rare opportunity for insureds over age 65 to purchase 20-year term insurance
- Cash value may accumulate in this product
- Dividends can be used for paid up additions or to reduce premium – dividends are not guaranteed

Best Interest Regulations – In February 2020, the National Association of Insurance Commissioners (NAIC) approved its Suitability in Annuity Transactions Model Regulation (#275) (the “Best Interest Regulation”). The Best Interest Regulation requires that all annuity recommendations by producers and insurers meet a new “best interest” standard. View and download the required forms, a copy of the applicable Best Interest Regulation for individual states, as well as training materials.
Ohio Insurance Guaranty Association Disclosure Statement (PDF) – This form is to be included with all OHIO applications. Please make sure you send a signed copy of this form with all OHIO life insurance and annuity applications.
MIB Notice to the Proposed Insured (PDF) – The MIB Authorization form has changed. The new form will take effect on January 1, 2013. This new MIB Authorization form (form #: MIB-2012) must be included with all life insurance applications starting January 1, 2013. Any applications with the old MIB Authorization form (Form #: MIB 2009) received after 1/1/13 will be returned with the new form.
New York: MIB Notice to the Proposed Insured (PDF) – This form will also take effect on January 1, 2013. It must be included with all New York Life Applications starting January 1, 2013. Any applications received after 1/1/13 will be returned with the new form.

Non-qualified Section 1035 Tax Free Exchange Request
- For Life (PDF) – For Non-Qualified 1035 Exchange from a life policy at another company to a life or annuity policy at FCSLA Life. This form is required if the applicant is surrendering a policy with another company and using that surrender as a tax-free exchange to purchase a policy from FCSLA Life. This form must be submitted with the Replacement of Life Insurance or Annuities form.
- For Annuity (PDF) – For Non-Qualified 1035 Exchange from an annuity policy at another company to an annuity policy at FCSLA Life. This form is required if the applicant is surrendering a policy with another company and using that surrender as a tax-free exchange to purchase a policy from FCSLA Life. This form must be submitted with the Replacement of Life Insurance or Annuities form.
Qualified IRA Transfer Request (PDF) – This form is required if the applicant is transferring qualified monies (401K, IRA, Roth IRA, Simple IRA, SEP) and depositing to a qualified Annuity.
Generic Transfer Request (PDF) – For transferring funds from other than non-qualified annuities (i.e. bank account, CD, savings account, etc.).
HIPAA Authorization (PDF) – This authorization will be used to obtain medical records, if necessary, from the doctor(s) listed on the back of the form.
